چکیده

Background Tumors, including hematologic malignancies, can use the PD-1 pathway to evade immune surveillance. Pembrolizumab, a highly selective, humanized monoclonal antibody that blocks interaction of PD-1 with its ligands PD-L1 and PD-L2, has demonstrated robust antitumor activity and a manageable toxicity profile in advanced solid tumors. In earlier data, pembrolizumab demonstrated efficacy in Hodgkin lymphoma (HL)[1]. KEYNOTE-013 (ClinicalTrials.gov, NCT01953692) is a multicenter, nonrandomized, open-label, multicohort Phase Ib trial designed to assess safety and efficacy of pembrolizumab in patients with hematologic malignancies.

Pembrolizumab (MK-3475) in combination with lenalidomide and low-dose dexamethasone for relapsed/refractory multiple myeloma (RRMM): KEYNOTE-023

Background The immunomodulatory drug (IMiD) lenalidomide has single-agent activity against RRMM, and synergistic effects when combined with dexamethasone. Overexpression of PD-L1 is associated with tumor invasiveness in MM cells and may be a mechanism of immune evasion. Pembrolizumab, a highly selective, humanized IgG4 anti-PD-1 monoclonal antibody designed to block interaction of PD-1 with PD-...

Phase II study of pembrolizumab (MK-3475) for relapsed/refractory classical Hodgkin Lymphoma (r/r cHL): keynote-087

Background The prognosis is poor for patients with cHL who relapse after autologous stem-cell transplant (auto-SCT) or progress after brentuximab vedotin (BV) therapy. cHL frequently harbors genetic amplification at 9p24.1, which leads to overexpression of PD-L1 and PD-L2 on the tumor cell surface. This suggests that cHL may have a genetically determined dependence on the PD-1 pathway for survi...

Phase I Study of Pembrolizumab (MK-3475; Anti-PD-1 Monoclonal Antibody) in Patients with Advanced Solid Tumors.

PURPOSE This phase I study evaluated the safety, maximum tolerated dose, antitumor activity, and pharmacokinetics and pharmacodynamics of pembrolizumab in patients with advanced solid tumors. EXPERIMENTAL DESIGN In a 3 + 3 dose escalation study, 10 patients received pembrolizumab 1, 3, or 10 mg/kg intravenously every 2 weeks until progression or intolerable toxicity. Seven additional patients...

Preclinical to clinical translation of anti-PD-1 blockade

Background Pembrolizumab (MK-3475), a humanized monoclonal IgG4 antibody against programmed death receptor 1 (PD-1), is currently being studied in clinical trials across more than 30 types of cancers. Immunotherapy with anti–PD-1 monoclonal antibodies such as pembrolizumab shows robust, durable anti-tumor activity in multiple indications including patients with advanced melanoma. A more complet...
